Qualifying Life Event Enrollment Form
2024-2025 Academic Year
Students who do not enroll themselves or their dependents during the initial enrollment period can only enroll later if they experience a qualifying life event. They must complete the qualifying event enrollment form and provide supporting documentation within 30 days of the life event.
Qualifying life events may include:
Birth/Adoption– you may enroll a new dependent as of the date of birth or adoption. Documentation of the adoption or birth is required.
Entry into the U.S. – if your spouse or child(ren) are scheduled to move to the U.S. after the start of the student’s coverage, the student may enroll their dependent(s) in the insurance plan effective the date they arrive in the U.S.. A copy of the passport stamp with the date of entry or the dependents I94 is required to confirm the date of entry/effective date.
Marriage to the student – if the student is married after the effective date of their coverage, they may enroll their new spouse as of the date of marriage. A copy of the marriage certificate is required.
Postdoctoral Fellowship: If your postdoctoral fellowship began after the semester coverage period started, you can enroll your dependent from the date your coverage became effective. Upload a copy of the email from IU confirming your insurance eligibility start date along with your dependent enrollment form as your supporting documentation.
