Eligible Student Academic Appointees (SAA), Appointed Postdoctoral Fellows and Fellowship Recipients may elect to continue coverage for themselves and their dependents, including their spouse and children under age 26. 

The enrollment form will be available once IU notifies us that your enrollment under the SAA, Appointed Postdoctoral Fellowship and/or Fellowship Recipient plan ends. You have 30 days from the date the prior plan ends to complete this enrollment form.
