Insurance Eligibility

Student Academic Appointees (SAA), Appointed Postdoctoral Fellows and Fellowship Recipients who meet the minimum eligibility requirements are automatically enrolled in this plan each semester. The university pays the full cost for the student’s premium.

Eligible Appointees include:
  • Student Academic Appointees appointed at 37.5 FTE or greater
  • Appointed Postdoctoral Fellows, e.g. T-32 grants
  • Fellowship Recipients with an award of $6,750 or more per semester (Fall and/or Spring) and enrolled in at least six credit hours or the G901 thesis/dissertation equivalent each semester (summer excluded).

Those eligible may insure their dependents, including a spouse and/or children under age 26, by completing the dependent enrollment form on the left side of this page by the deadline. You are responsible for the full cost of their premiums. (* For School of Medicine Postdoctoral Fellows, dependent premiums for all eligible dependents are fully paid by the university.)

Plan Highlights

  • The standard plan year begins August 1 and ends July 31 the following year
  • The Plan includes comprehensive medical, prescription, vision, and dental coverage for one premium
  • The medical plan uses the Anthem Blue Access PPO network with access to providers throughout Indiana, nationwide and overseas.
  • Plan restarts annually on August 1
  • Annual deductible is $0 per member / $0 per family for in-network coverage 
  • Out-of-pocket maximum is $2,500 per member / $5,000 per family for in-network coverage
  • In-network preventive care is covered by the Plan at 100% 
  • IU Student Health Center office visit copay is $15
  • Primary Care office visit copay is $25; Specialist office visit copay is $35 
  • Urgent Care copay is $50; Emergency Room copay is $100
  • Most other services are covered with 20% coinsurance after deductible 
  • Medical coverage includes therapy services such as chiropractic care, physical therapy, behavioral health, mental health, and substance use disorder services; See student brochure or plan booklet for more information 
  • Vision care coverage through Anthem Blue View Vision, including an annual eye exam with a $20 copay and discounts for vision wear 
  • Dental care coverage through Anthem Blue Cross and Blue Shield Dental Complete, includes diagnostic and preventive services covered at 100% when using in-network providers

Additional Services and Benefits

  • Sydney Health app to access your Anthem ID card, find a provider, see claims, and more
  • Your medical coverage follows you wherever you go, including nationwide and overseas. As an Anthem member, you have access to care across the country through the BlueCard PPO program, and outside the U.S. through the BCBS Global Core program. Visit the Student Travel Coverage page in the left-hand menu to learn more, including how to find doctors and how to access emergency or non-emergency healthcare while traveling.
  • Your health plan includes access to emergency travel assistance through Academic Emergency Services (AES). With AES you have access to 24/7 support if you experience an emergency while traveling over 100 miles from home or outside your country. Services include emergency medical evacuation and repatriation, pre-travel and safety assistance, and an accidental death & dismemberment (AD&D) benefit. Student Enrollment & Premiums

As an eligible student you are automatically enrolled in the plan each semester. Participation is mandatory unless proof of comparable coverage and a waiver form are submitted and approved by the deadline (see Waiver Deadline and Waiving Coverage section below).

1 Enrollment is automatic. All costs are paid by the University.
Coverage is effective:
  • SAAs & Postdoctoral Fellows are covered effective their first day of active employment. However, coverage will not be activated until their position has been updated in the university system.
  • Fellowship Recipients are covered for the entire semester that they are eligible. However, your coverage will not be activated until your eligible award is posted and enrollment in minimum credit hours has been completed.

Any delay may result in you paying for healthcare costs out-of-pocket and then submitting claims for reimbursement once your coverage is activated.

Your coverage will terminate when you cease to be a member of the eligible class for coverage.

Dependent Eligibility, Enrollment & Premiums

As an eligible student, you have the option to insure your spouse and unmarried children under the age of 26 for an additional cost. Coverage is only available to your dependents if and when you are insured under the plan. To cover your dependent(s) on the plan, complete the Dependent Enrollment Form from the left-hand menu.  Your application and premium payment must be completed by the deadlines found in the chart below. Dependent premiums can be paid in full at the time of enrollment or by monthly installments; however, they cannot be charged to your bursar account. Once enrolled, no portion of the premiums are eligible to be refunded.

If the enrollment deadline has passed, you cannot enroll your dependent(s) until the next coverage period, unless you or your dependent have experienced an IRS-qualifying life event (i.e., marriage, birth of a child, loss of prior coverage). If your dependent experiences a qualifying life event you will need to submit a Qualifying Event Enrollment Form from the left-hand menu and submit supporting documentation within 30 days of the event. 

Dependent coverage ends on the date the dependent no longer meets the criteria for dependent eligibility or the student ceases to be a member of the eligible class for coverage.

1 Dependent premiums are separate from the student’s premiums. The premium amounts listed above are for the additional dependent coverage only. Dependent premiums cannot be added to the student’s bursar account and must be paid directly to UHP. Premiums can be paid in full at the time of enrollment or by monthly installments.

Waiving Coverage

Students with other insurance may opt out of the SAA and Fellowship Recipient Insurance plan. Before waiving, compare this plan's cost, coverage, and benefits to your existing plan. The IU plan provides comprehensive coverage throughout Indiana, nationwide and overseas; not all personal plans will provide comprehensive coverage. 
If you are waiving the SAA and Fellowship Recipient plan in order to be enrolled on another IU student insurance plan you must take further action to be enrolled on those plans.
  • To enroll in the Professional Insurance plan, contact the IU Student Insurance office at studenhc@iu.edu.
  • To enroll in the International Student Insurance plan, you must inform the international office on your campus that you need to be enrolled.

To waive this plan you will need to provide documentation from your health insurance plan. Please have a copy of your insurance ID card and a copy of your plan summary available to provide information on the waiver form and to download.

Once you opt out of the plan, you cannot voluntarily re-enroll later unless you experience an IRS qualifying life event and qualify for special enrollment rights.

By waiving this plan you will be certifying that the coverage under your health plan is comparable to coverage under the Student Health Insurance Plan, and you understand that you are responsible for your medical expenses.  You will also certify that your insurance coverage will remain in effect, providing coverage in the geographic area surrounding the Indiana University campus during the 2024-2025 academic year.  The submission of your waiver form, including all information included, constitutes truthful and accurate statements.

A new waiver is required to be submitted each academic year. 

Qualifying Life Event

Students who do not enroll themselves or their dependents during the initial enrollment period can only enroll later if they experience a qualifying life event. They must complete the qualifying event enrollment form and provide supporting documentation within 30 days of the life event.

Qualifying life events may include:

Loss of other coverage – if your other insurance coverage ends you may be eligible to enroll in IU’s insurance plan. Documentation supporting the loss of your other coverage is required.

Birth/Adoption– you may enroll a new dependent as of the date of birth or adoption. Documentation of the adoption or birth is required.

Entry into the U.S. – if your spouse or child(ren) are scheduled to move to the U.S. after the start of the student’s coverage, the student may enroll their dependent(s) in the insurance plan effective the date they arrive in the U.S.. A copy of the passport stamp with the date of entry or the dependents I94 is required to confirm the date of entry/effective date.

Marriage to the student – if the student is married after the effective date of their coverage, they may enroll their new spouse as of the date of marriage. A copy of the marriage certificate is required.

All qualifying life events must be submitted within 30 days of the event.

Insurance ID Cards

After enrollment, you’ll receive an email from Anthem Student Advantage when your digital ID card is ready to download. You can access your ID card by downloading the Sydney Health app or visiting Anthem.com. You can then email or fax it from your smartphone or computer or request a hard copy ID card be mailed to your address on file. 

Continuation Opportunities

When your position or appointment with the University ends, you will have the opportunity to continue your coverage for yourself and your dependent(s) for up to 3 months.  You must elect coverage within 30 days of your loss of coverage.  You can only make one election for either 1, 2 or 3 months at the time of enrollment.  If you choose one month of coverage, you cannot re-enroll for additional months at a later time. Payment can be made by credit card at the time the continuation enrollment form is submitted.
