Seton Hall University
STUDENT HEALTH INSURANCE PLAN
Annual Waiver Appeal Form
2025-2026 Academic Year
You may only waive the plan if you have an active comparable health insurance that meets all waiver requirements. Seton Hall requires that students be insured on a US health insurance plan that provides them with both emergency and non-emergency and non-emergency access to doctors, specialists, hospitals, labs, pharmacies and mental health/behavioral health providers in the South Orange, New Jersey campus area without restriction or limitation. Emergency/Urgent Only coverage in the area of the campus does not meet the insurance requirements for a waiver. Plans must be filed and approved in the USA and fully compliant with the US Affordable Care Act.
The waiver deadline was on September 5th and has now passed. All students who did not take action have been automatically enrolled and may submit a waiver appeal form through October 15th. If you have utilized the coverage prior to submitting your waiver appeal, your waiver will not be approved and you will remain enrolled and responsible for the premium.
