Spring 2025
Only returning students enrolled in coverage starting in August 2024 should use this form. If Spring 2025 is your first semester of the academic year and you are being charged for the plan, please use the waiver form instead.
If you are not returning to WFU for the spring semester, you are not re-enrolled in coverage for the spring semester and your last day of coverage is 12/31/2024.
To opt-out for spring, students must have insurance through an ACA-compliant plan that is filed and approved in the U.S. The plan must offer comprehensive, non-emergency coverage for doctors, specialists, hospitals, labs, pharmacies, and behavioral health providers in the area surrounding the school.
- Generally, only plans acquired through U.S. employment or purchased from the local state marketplaces will be filed and approved in the U.S. and compliant with the ACA. Short-term medical plans, healthcare sharing plans, insurance plans from foreign healthcare systems, travel plans, and plans marketed to international students only do not meet the waiver requirements.
- Not all U.S. health plans provide national coverage, and many out-of-state HMO, EPO, and Medicaid plans will not provide comprehensive non-emergency benefits outside of the home coverage area.
