University of Massachusetts - Boston
Student Health Insurance Plan
2024-2025 Policy Year Waiver Form

All full-time and three-quarter time graduate and undergraduate students must either waive or enroll in the UMass Boston Student Health Insurance Plan (SHIP) at the beginning of each academic year.

A waiver form completed in the fall semester will be automatically applied to your student account for the spring semester of the same school year. No additional action is necessary in the spring. Eligible fall students who miss the fall waiver deadline will not have access to the spring waiver form and will be responsible for the fall and spring insurance charge.

This is the only way to request the removal of the 2024-2025 UMass Boston SHIP charge from a student's account. Submitting a copy of your insurance ID card through the Health Services Portal does not satisfy the requirement to waive the SHIP.

Your health insurance plan must meet the below requirements to waive the Student Health Insurance Plan:

  • Your plan must provide access to doctors, specialists, hospitals, labs, pharmacies, and mental health/behavioral health providers in Massachusetts, specifically the UMass Boston campus area. Please note, if you are enrolled in an out-of-state Medicaid, HMO, or EPO your coverage may not be sufficient in Massachusetts. Many out-of-state plans do not provide comprehensive benefits outside of the home service area, and urgent or emergency-only coverage in Massachusetts is not sufficient and does not meet the SHIP waiver requirements.
  • MassHealth Limited, Health Safety Net, and Children’s Medical Security Plan do not meet the insurance requirements and are not accepted.
  • Be a US health insurance plan that is filed and approved in the US. Travel insurance and short-term limited duration plans are not accepted.
  • Provide coverage for the entire policy period with no break in coverage.
  • Comply with the Affordable Care Act (ACA) and provide minimum essential coverage as defined by the ACA including:
    • Provide an unlimited policy maximum
    • Include coverage for pre-existing conditions
    • Provide emergency and non-emergency care in the area of the University
