The 2026-2027 fall waiver deadline was October 1, 2026. All students who did not take action prior to the deadline have been enrolled and charged for the plan and are responsible for the premium. LATE WAIVERS ARE NOT ACCEPTED. To request a spring termination form to opt-out for the spring, please reach out to info@univhealthplans.com.

The Spring 2027 waiver form for new spring students will be available later this winter.

Students who did not submit a waiver are eligible to submit a Spring termination request form later this fall. Please reach out to info@univhealthplans.com if you missed the fall deadline and would like to waive for the spring.
