The 2025-2026 annual waiver policy period waiver deadline was October 1, 2025. All students who did not take action prior to the deadline have been enrolled and charged for the plan and are responsible for the premium.
The Spring 2026 waiver form will be available to new spring students in early-December.

*The plan deductible DOES NOT APPLY to in-network Primary Care Visits, Specialists, X-ray, Blood Work (Labs), Imaging (CT/PET Scans, MRIs), Prescriptions, Emergency Room Visits, Urgent Care and Emergency Transportation, Outpatient Mental and Behavioral Health, Prenatal Care and Outpatient Rehabilitation.
