Use this form to request a change of address for your Blue Cross Blue Shield Student Health Insurance Plan. You should also update your mailing address with UMass Boston, if you have not already done so.

A new ID Card will not be issued following completion of this form. You can access a digital ID card  24/7 using the Insurance ID Card link located in the navigation column on the left hand side of the page. To request a new mailed ID card, please email info@univhealthplans.com, after submission of this form.

If you do not have a U.S. mailing address you do not need to complete this form and your mailing address will be defaulted to the UMass Boston Bursar's Office.
