The 2024-2025 Spring dependent enrollment form is closed. The dependent enrollment form for 2025-2026 annual students will be available in July.

If your dependent has a qualifying event, such as a loss of other coverage, they may qualify for special enrollment rights. Please contact University Health Plans at 800-437-6448.
