Early Termination Form
2024-2025 Academic Year
PLEASE READ: This form should ONLY be used to terminate coverage resulting in a partial enrollment in the 2024-2025 SHIP.
Students enrolled in the 2024-2025 Student Health Insurance Plan (SHIP) can use this form to terminate their coverage mid-academic year if they are in one of the following situations:
- not returning to Yeshiva or Cardozo School of Law for the spring semester
- enrolled in a new comprehensive health insurance plan with an effective date after 08/15/2024
Students requesting termination for the entire semester should use the waiver form on the left.
