The 2016-2017 Brochure is not yet available. You may refer to your 2015-2016 campus brochure as the benefits will remain the same with the exception of the Outpatient Physician's and Consultant's copay per visit has been changed from $15 to $20 per visit; and the Outpatient Physiotherapy copay per visit has been changed from $15 to $20 per visit. In addition, the Out-of-Pocket Maximum has been changed from $1,000 to $6,850 per Insured Person, Per Policy Year.
All undergraduate day students, both domestic and international, all international graduate students, and any graduate student required to be enrolled under programmatic requirements (e.g., students enrolled in the Physician Assistant Program, on-campus Occupational Therapy Program, Addiction Counseling or Clinical Mental Health Counseling master's degree programs, etc.), registered and attending Johnson & Wales University and taking credit hours (excluding full-time Johnson & Wales employees) are eligible and enrolled in the plan on a hard waiver basis. Whether a student is "registered" and "attending" shall be determined exclusively by Johnson & Wales University. Students MUST maintain eligibility each semester to remain enrolled in annual coverage.
NO ACTION IS REQUIRED OF YOU TO ENROLL IF YOU MEET THE ABOVE ELIGIBILITY.

Before waiving, we encourage students to compare their insurance
benefits to the JWU Student Health Insurance Plan (SHIP).

If you meet the eligibility criteria and wish to enroll in the SHIP, no action is required. Eligible students are automatically charged and enrolled.
