Spring Enrollment Form

Although students are automatically charged on their tuition statement for the SHIP, student information is not transferred to UHCSR until after the deadline. Students should submit an enrollment form as soon as possible to avoid difficulties accessing their health insurance benefits.
Starting in December, please allow up to 4 business days after submitting the enrollment form for the student information to be active in the insurance company's database. Students who do not submit an enrollment or waiver form by the deadline will be enrolled in the SHIP retroactively to the policy effective date and responsible for the insurance premium.
Students may also enroll their eligible dependents. Dependents include the student's legal spouse and dependent children under age 26. Dependent premium is paid directly to UHP and cannot be added to the student tuition statement.
Eligible students must be logged into their Saint Joseph's University Portal to access the enrollment form.
