The deadline to confirm enrollment for the spring 2025 policy period has passed. The form is no longer available. If you are an eligible new student for spring 2025 and did not fill out the waiver form by the deadline you will be automatically enrolled in the Student Health Plan effective January 15, 2025 and responsible for the insurance charge on the student account.
