The deadline to submit an enrollment confirmation form for the 2024-2025 policy period was October 8, 2024. 

Eligible students who did not submit a waiver or enrollment confirmation form by the fall 2024 deadline have been automatically enrolled in the Plan retroactive to August 1, 2024 and are responsible for the insurance charge on the student account. If you return as an eligible spring student you will be automatically charged and re-enrolled for the spring 2025 coverage period. No action is needed.

NEW SPRING STUDENTS ONLY: The spring 2025 policy period enrollment confirmation form will be available in early January 2025. Please check back for updated information.

If you previously waived and have experienced a qualifying life event, please contact University Health Plans to discuss eligibility to enroll in the Student Health Plan.
