2024-2025 Summer Waiver Form

According to the Commonwealth of Massachusetts and MCPHS University policy, all students (regardless of enrollment) must be covered by a comprehensive health insurance plan which meets these standards. The University makes available a health insurance plan which meets these standards. To waive coverage, please provide the information below.

The waiver form deadline is May 1, 2025.
