The fall 2024 waiver deadline has passed.

If you have a life changing event (i.e. involuntary loss of coverage) and need to enroll in the insurance plan after waiving, please contact University Health Plans at 1-833-251-1148 or info@univhealthplans.com. Enrollment must occur within 30 days of the qualifying life event.

If you have a true extenuating circumstance for missing the initial waiver deadline you may select "Waiver Appeal Form" on the left and submit that form for review. This is not a guarantee of a waiver. The waiver appeal deadline is October 22, 2024. No exceptions will be made after that date.

The spring 2025 policy year waiver form will be available early December 2024.
