The enrollment form for Spring 2025 is closed. Students who did not complete a form have been automatically enrolled and charged the student health insurance premium.

To access your online insurance card, please go to www.universityhealthplans.com/montclair and select Insurance ID Card on the left side of the page.

The enrollment form for Fall 2025 will be available in mid-July.
