Spring Enrollment Form

All Domestic Graduate, Undergraduate and Part-time Students taking 3 or more credit hours are required to purchase this insurance plan unless proof of comparable coverage is furnished by submitting a waiver form. All International Students are required to enroll in the Plan and may not waive.

Enrollment in the Student Health Plan will not be processed until after the posted deadline. To expedite the processing of your enrollment, St. John's College requires student to complete this enrollment form. The student account will be billed the student only annual insurance fee.

The deadline to submit the enrollment form for the spring 2025 policy period has been extended to January 6, 2025
