The deadline to confirm enrollment for the 2024-2025 policy year was June 10, 2024. The form is no longer available. If you are an eligible student and did not submit the online waiver form or enrollment confirmation form by the deadline, you will be automatically enrollmed in the Student Health Plan and responsible for the charge on the student account.

If you have experienced a loss of other coverage after the deadline. Please contact Student Financial Services to request enrollment in the Student Health Plan within 60 days of losing your other coverage.
