OPT Student & Dependent Enrollment Form

OPT Students may enroll themselves and their eligible dependent(s) in semester-based coverage. If you do not enroll yourself or your dependent(s) by the enrollment deadline, you and your dependent(s) can only enroll later if you experience an IRS-qualifying life event.

If you did not enroll yourself or your dependent(s) during the semester’s enrollment period, you may be eligible for a special enrollment period if you or they experience an IRS-qualifying life event.
