STUDENT HEALTH INSURANCE PLAN
2024-2025 Academic Year
St. John’s College is pleased to present you with information about our 2024-2025 Student Health Insurance Plan (SHIP). The insurance coverage is through Wellfleet with University Health Plans (UHP) providing administrative support for the waiver and enrollment forms, customer service and general insurance questions.
ELIGIBILITY
All Domestic Graduate, Undergraduate and Part-time Students taking 3 or more credit hours are required to purchase this insurance plan unless proof of comparable coverage is furnished by submitting a waiver form. All International Students are required to enroll in the Plan and may not waive. Insured Students who are enrolled in the Student Health Plan may also enroll their eligible dependents.

DOMESTIC STUDENT WAIVER PROCESS
Students who have an insurance plan that meets the below waiver requirements set by the College may elect to waive the Student Health Insurance Plan by completing the waiver form located in the left navigation menu of this page.
FAILURE TO COMPLETE the online Waiver Form by the deadline will result in your mandatory purchase of the College’s Student Health Insurance Plan for the 2024-2025 policy year, without the possibility of a late waiver, refund or cancellation.
To waive the SHIP your health insurance plan must meet the below requirements:
- Be a health insurance plan that is filed and approved in the United States;
- Provide coverage for the entire academic year with no break in coverage
- Comply with the Affordable Care Act (ACA) and provide minimum essential coverage as defined by the ACA including:
- Provide an unlimited policy maximum
- Include coverage for pre-existing conditions
- Provide emergency and non-emergency local coverage in the area of the College (the plan cannot limit coverage to emergency conditions only while in the area of the College. It must provide all of the coverage described above while in the area of the College.)
ENROLLMENT PROCESS
International Students: Enrollment is mandatory for all International Students. All international students are automatically enrolled in the College’s Student Health Insurance Plan, and may not waive out of it. To expedite enrollment in the Plan, students may confirm enrollment by submitting the enrollment form located in the navigation menu on the left side of this page.
Domestic Students: Eligible students enrolling in the SHIP must confirm enrollment by submitting the online enrollment form located in the navigation menu on the left side of this page. Eligible students who fail to complete the on-line waiver or enrollment form by the deadline will be automatically enrolled in the SHIP for the entire policy year and will be billed accordingly. Waivers will not be accepted after the deadline. Completion of the on-line enrollment or waiver form is required annually.
Dependents: All students enrolled in the SHIP may enroll their eligible dependents on to their Plan. Dependent enrollment election can be made when submitting the online enrollment form. The dependent premium cannot be added to your student tuition bill and must be paid for at the time you submit the form. Dependent enrollment must be elected prior to the enrollment deadline. Beyond the deadline, dependents may only be added if due to a qualifying life event.
Re-enrollment after waiving: If you have a life changing event (i.e. involuntary loss of coverage) and need to enroll in the insurance plan after waiving, please contact University Health Plans at 1-833-251-1136 or info@univhealthplans.com. Enrollment must occur within 60 days of the qualifying life event.
