This form is used to enroll dependents in semester-based coverage. If you do not enroll your dependent(s) by the enrollment deadline, you can only enroll your dependent(s) later if they experience an IRS-qualifying life event.

IEP & PIE Dependent Enrollment

IEP and PIE students should use this form to enroll their dependents.

Qualifying Life Event Enrollment

If you did not enroll your dependent(s) during the semester’s enrollment period, they may be eligible for a special enrollment period if they experience a qualifying life event.
