Step 1: Confirm you have selected the Blue Cross Blue Shield of Massachusetts Formulary. (You may be prompted to make a selection before reaching this page.)
 

Step 2: Search for your medication by name. After you select your medication, confirm you have selected the 5-Tier Plan on the right. For medication searches prior to August 1, 2022, choose the 3-Tier Plan.
