The Termination Request Form is only for students who are currently enrolled in the Student Health Insurance Plan (SHIP).

To be eligible to terminate your plan, you must be enrolled in other coverage. If you are currently enrolled in the SHIP and are enrolled/enrolling in other coverage before the start of the next semester, click here for the Termination Request Form.

The Termination Request Form is only for students currently enrolled in the SHIP. If this is your first semester at MGHIHP, please go back and choose the regular waiver form.
