QUALIFYING EVENT ENROLLMENT FORM
2024-2025 Policy Year

The Qualifying Event Enrollment Form is only for students who waived the 2024-2025 Student Health Insurance Plan (SHIP).

To be eligible for a qualifying life event enrollment, your other insurance must be ending or have ended within the past 60 days. If you are losing/lost your other insurance, click here for the Qualifying Life Event Enrollment Form.

The qualifying event enrollment form is only for students who previously waived the 2024-2025 SHIP and qualify for a special enrollment period. If you previously waived, and do not have a recent loss of other coverage, you cannot enroll until the 2025-2026 academic year. If this is your first semester at MGHIHP, please go back and choose the regular enrollment form.
