2012-2013 STUDENT HEALTH INSURANCE PLAN

The fall 2013 and spring 2014 waiver form will be available beginning June 1st, 2013.

Dear SVA Student,

SVA requires all registered, matriculating students to have health insurance coverage for both in- patient and out-patient medical care. Students must provide information about their insurance coverage each year. All students are automatically enrolled in the SVA - sponsored Student Health Insurance and billed by Student Accounts for the Health Insurance Fee. Students who have outside coverage and do not want the SVA-sponsored coverage must provide information about their insurance coverage each year.

Please take a few minutes to consider your enrollment in the SVA - sponsored Student Health Insurance. Please refer to the plan brochure by clicking on the “Brochure” link on the left hand side of this page.

This plan would cover you from January 2, 2013 through August 24, 2013. Below are some of the highlights of the School of Visual Arts Student Health Insurance Plan:

  • Access to United HealthCare's nationwide provider network
  • $150,000 maximum benefit per plan year
  • 90% coverage for Preferred Care / 60% coverage for Non-Preferred Care
  • Prescription drug coverage
  • Mental health coverage
  • Inpatient hospitalization coverage
  • Travel assistance services

Once you have reviewed the information, please make your insurance waiver selection here by choosing the “Waiver Form” link on the left hand side of the page, follow the directions and provide the requested information.

Students who elect to waive the SVA- sponsored Health Insurance Fee must submit a waiver online to provide proof of comparable coverage by February 6, 2013.  If you do not complete a waiver by the deadline date, you will be responsible for the Health Insurance Fee billed by Student Accounts.

The waiver deadline for the spring 2013 semester is February 6, 2013. If you have a life changing event (i.e. involuntary loss of coverage) and need to re-enroll in the SVA – sponsored Student Health Insurance after submitting a waiver, please contact the SVA Office of Student Health and Counseling Services at 212-592-2246 to complete an Enrollment Form.
