Dear Students,
Welcome to the Bay State College Student Health Insurance Plan On-line Waiver/Enrollment System.
On this site you can find links to the Student Accident & Sickness Insurance Plan Brochure, Provider Search, On-line Waiver Form and On-line Enrollment Form. You can access The Waiver Form or Enrollment Form by clicking on the appropriate box to the left. However, it is VERY important that you review the information below before proceeding to the waiver form.
Full Time or Three-Quarter Time Students at Bay State College, who are enrolled in a health care plan or who are covered by a parent/spouse’s health care plan, which meets the minimums set forth in Massachusetts Law C.15A, s. 18 (available on the http://mass.gov website; type QSHIP into the search box), use the Waiver Form found on this site to waive for the 2013-2014 Student Health Insurance Plan. The Student Health Plan for the 2013-14 Policy Year is insured by National Guardian Life Insurance Company with Consolidated Health Plans paying claims. The premium is $1,184 for the Policy Year and the policy is in effect from September 1, 2013 through August 31, 2014. The premium for the Spring Only plan is $837 for the period of January 14, 2014 through August 31, 2014 and the Summer Only Plan is $475 for the period of May 5, 2014 through August 31, 2014.
Bay State College does not allow the following classes of students to waive off the QSHIP plan. If the student falls under one of the following categories they are ineligible for waiver and will be automatically enrolled:

student with coverage from insurance carriers outside the U.S. and/or coverage by foreign National Health Services programs;
-
students with a health insurance plan that provides coverage through a closed network of providers not reasonably assessable in the area where the student attends school, for all but emergency services; or
-
if you are currently covered by the Commonwealth Care Plan of Massachusetts you are required by the state to notify them of your change to full time or three quarter time student status, which does not meet the eligibility requirements of the Commonwealth Care Plan.
Please note, in addition, the following students are not eligible to use this on-line waiver system:

Students with Travel Insurance or a Commonwealth Care Plan may not waive.
Very specific information is necessary to complete the waiver form. Please be sure to have your insurance card available before completing the waiver form.
